INSURANCE, BILLING & YOUR OUT-OF-POCKET COSTS
Because of the hospital-based model, your visit can include separate charges:

- A Facility (Hospital) Charge for use of the hospital-based outpatient department.
- A Provider (Physician) Charge for the clinical care.
These two charges may affect your out-of-pocket costs differently depending on your plan’s copays, deductibles and coinsurance. We encourage you to review your benefits and contact our billing team with any questions.
